- Diagnosis and current clinical status
- Referring provider and facility
- Requested treatment or service
3. Send us the following required documents by fax - 415-461-8158:
- Discharge summary
- Patient Face Sheet
- History and physical
- Imaging and lab results
Once your patient is accepted for transfer to MarinHealth Medical Center, you'll need to download and complete the following transfer forms and return them to us by fax: 415-461-8158.
