OB Pre-Registration

We strongly encourage all moms-to-be who want to deliver at MarinHealth Medical Center to pre-register during their second trimester. This ensures they will be admitted as rapidly as possible when they arrive in labor. To pre-register, download our OB Pre-Registration form in .PDF format and fax the completed document to our office: 415-925-7241.

Many insurance companies require pre-certification for hospital admissions. Check with your insurance carrier for more information. Failure to obtain pre-certification may result in your insurance company reducing the benefit paid, making you responsible for a greater portion of your hospital bill.

Please bring your state or federally issued identification card and insurance card when coming to the hospital.
