Make an Appointment

To make an appointment for a consultation, follow-up visit, or bone density screening, please call 1-415-461-1780. Our receptionists are available Monday to Friday from 9:00 am to 12:00 pm and 2:00 pm to 5:00 pm. If our office is closed, or our staff is assisting other patients, you may need to leave a message and we will get back to you in a timely manner. For medical emergencies, please call 911.

First Consultation

The initial consultation takes about 45 minutes and will include a review of your health history, past medical records, a physical exam, discussion, and a health plan. Patients should plan on arriving 10 minutes before the scheduled visit to check in with our office staff. Health History forms are available online or can be mailed to you upon request.

For Your Appointment

It is helpful to bring any prior medical records to your upcoming appointment or to have your referring physician fax pertinent history and lab results to our office. You should also bring your insurance information. If you have changed your policy, please make sure to let us know at your next visit. Helpful insurance information for you to obtain prior to your appointment may include:

  • Required referrals and authorization for services rendered
  • Co-payment and deductible amounts (you may have a co-pay for office visits but a deductible for labs and procedures)
  • Covered laboratory services as well as procedures
  • Claims address

Cancellation Policy

Our cancellation policy requires 24 hours notification to permit rescheduling. You may be charged a fee if an appointment is missed without 24 hours’ notice or a reasonable excuse. Your cooperation is appreciated.

After Hours

After-hours coverage is provided evenings and weekends by the physician on call. For situations that cannot wait until regular office hours, the physician on call can be paged through our voicemail system. We appreciate our patients limiting these calls but also encourage you not to overlook a medical emergency for fear of "bothering" the doctor on call. We are here to care for you.
