At the Braden Diabetes Center, our knowledgeable certified diabetes educators, diabetes nurse specialists, nurse practitioners and registered dietitians are available to help you find the best way to manage your diabetes. Below, you will find a list of services offered by our center.
Diabetes Self Management Education (DSME)
Learn the most current and effective ways to manage your diabetes in our
ADA certified curriculum DSME classes or in a one-on-one appointment,
depending on your personal needs and goals.
Medical Nutrition Therapy (MNT)
Work individually with one of our outstanding registered dietitians to
learn the impact your food choices have on your body and on managing your
blood sugar. Get personalized assistance with meal planning based on your
individualized goals.
Glycohemoglobin A1c Testing
Receive an update on your average blood sugar levels over the past three
months. This important piece of information leads to better blood sugar
management.
7 or 14 Day Professional Continuous Glucose Monitoring (CGM) Placements
Wearing a Continuous Glucose Monitor for 7 or 14 days gives us a deeper
look into your blood sugar patterns each day and over time. This valuable
information will enable our staff and your primary care physician (PCP)
or endocrinologist help you to achieve improved blood sugar management.
Personal Continuous Glucose Monitoring (CGM) Set Up and Glucose Trend Analysis
Our staff can help you get started with your own personal Continuous Glucose
Monitor (CGM) system. We can assist you in learning to optimize the use
of your CGM, including sharing your data with your diabetes care team
and loved ones. We can teach you how to use this valuable information
to fine tune your diabetes regimen and achieve tighter blood sugar levels.
Diabetes Technology Education and Assistance
We all know how intimidating new technology can be! Let our knowledgeable
staff show you the ins and outs of your new devices, apps and software
that make managing your diabetes easier.
Glycemic Control Evaluation and Management
Work with one of our certified diabetes educators to assess your current
state of diabetes control and learn how to fine tune your blood sugar regimen.
Insulin Therapy Education and Management
Staff at the Braden Diabetes Center are available to help you get comfortably
started on your new insulin regimen, and also available to assist your
PCP or endocrinologist in managing your regimen when requested.
Insulin Pump Education and Management
Our certified diabetes educators are here to educate you about all available
insulin pump options on the market and also to start you on your new or
upgraded insulin pump. We will teach you how to best manage your basal
and bolus insulin with insulin pump therapy.
Diabetes Limb Wellness
In collaboration with Bay Area Foot Care, the Braden Diabetes Center offers
podiatry care specifically tailored to diabetes patients who are at high
risk of dangerous foot issues such as ulceration. Our onsite podiatrist
provides podiatry screening and basic podiatry services.
Pre-Surgical Diabetes Planning
Having an elective surgery in the next few weeks or months and need help
achieving tighter glucose control to be best prepared for an optimal recovery
and outcome? We can help!
Diabetes Prevention
Learn strategies to help prevent diabetes, achieve meaningful weight loss
and live a healthier life.
Support Groups
Meet others with Type 1 or Type 2 diabetes and learn tips and tricks for
