Preparing For Your In-Person Visit

Before the Consultation

To help you prepare for the consultation, download and complete the self-assessment questionnaire to the best of your ability and bring it with you to the consultation.

The Day of Your Consultation

Arrive 15 minutes early to register and keep the following in mind:

  1. Prior to your visit, you will receive an email with information on what will be required of you.
  2. If you are a first time patient, please bring your completed self-assessment questionnaire.
  3. There is a limit to one person in the waiting room at a time. If you require assistance, your accompaniment is permitted to stay with you.
  4. Masks and social distancing will be enforced. If you are not wearing a surgical mask, you will be asked to change to the surgical mask provided. Failure to adhere to these rules will result in refusal of the appointment.
  5. Your temperature will be taken.
  6. You will go over your information to make sure it is up to date.
  7. Lastly, you must fill out paperwork: covid-19 screening and consent forms. If you are a Medicare patient, there will be additional paperwork.
  8. All patients should bring their blood glucose meter and log book (if you use these items), their state-issued photo ID, and insurance cards
