Lactation Services Pre-registration

To schedule an outpatient (postpartum) appointment with MarinHealth Lactation Services, please call 1-415-925-7522.

After you call and schedule your appointment, you will need to fill out the following pre-registration form if you did not deliver at MarinHealth Medical Center or UCSF Medical Center.

Many insurance companies require pre-certification for hospital admissions. Check with your insurance carrier for more information. Failure to obtain pre-certification may result in your insurance company reducing the benefit paid, making you responsible for a greater portion of your hospital bill.

Please bring your state or federally issued identification card and insurance card when coming to the hospital.

Please note: The following form should take about 15 minutes to fill out, and must be completed in one session.
