Thanks to our alliance with UCSF Health, MarinHealth Medical Network is able to provide patients with complimentary access to UCSF’s MyChart, a confidential portal that allows you to access your medical information online. MyChart uses secure technology, so messages to and from your provider remain confidential.

NEW: Easy Access for Parents via MyChart

Parents and guardians are now able to request proxy access to Child or Adolescent MyChart accounts completely online through MyChart. Proxy access allows parents and guardians to view minor's health information, update insurance and utilize self-service tools such as eCheck-In (features available by age). How to Request a Copy of Your Medical Records:

The easiest way to access your medical records is through the MyChart app. For those who would like a copy of their records mailed to them, first, please print and complete the MarinHealth Medical Network Medical Record Release Form (English | Spanish). The release form must be completed, dated and signed and a valid color copy of your photo ID attached. We ask that you specify what components of your medical records you wish to obtain. Often, the discharge summary, operative report and history and physical contain relevant information to suit your needs.
