Influenza Self Assessment

MarinHealth is concerned about the spread of flu this season, and care deeply about the well-being of our patients, their loved ones and our staff. That’s why we are taking active steps to prevent the spread of flu. For everyone’s safety, we ask that you review the chart below and conduct a self-assessment before entering the hospital. If you answer “Yes” to any of the items below, please postpone your visit.

• Have you had any of the following symptoms in the past week? (Yes/No)
  • Fever or feeling feverish/chills
  • Cough
  • Runny nose
  • Sore throat
  • Sinus congestion/Stuffy nose
  • Vomiting
  • Diarrhea
• Have you been in contact with anyone with the following in the past week? (Yes/No)
  • Flu
  • Cold

If you are a patient with a medical appointment such as lab, radiology or endoscopy, and have symptoms, please put on a mask, clean your hands often, and let registration know you are sick.
